Privacy and Confidentiality
The information you will provide will be treated confidentially, unless it must be breached for reasons of safety, such as a risk of you harming yourself or others. This would usually only be done with your knowledge. It is often advantageous to mental health care for there to be communication between different professionals and support people nominated by patients. This would occur with the patient’s permission and involvement. This Clinic complies with government privacy legislation. In signing this consent form, you authorise our doctors at South Eastern Specialist Centre to obtain copies of letters, reports and investigation results and to correspond with or discuss your treatment plan with other treating health professionals as clinically appropriate.

Your time and our time is valuable
We will attempt to send you an SMS reminder for your upcoming appointment. Please respond Yes to this SMS or call us immediately to reschedule. If you need to cancel, we require a minimum of 48 hours’ notice. If you provide less than 48 hours’ notice you will be charged a fee of 50% of your appointment fee.
If you do not attend an appointment you will be charged a fee of 100% of your appointment fee. This non attendance fee will be due on the day of your missed appointment and we will not rebook any future appointments until this has been paid in full.
Please understand that we have a waitlist for appointments and when an appointment is left unattended, we miss an opportunity to see a patient in need of this time.
Please note that if your appointments are paid for by a third party – such as DVA or TAC – these organisations do not pay for late-cancelling appointments or Failed-to-attend appointments, therefore if these fees are incurred they will need to be paid for directly by the patient.
Our Practice endeavours to run on time for appointments as much as possible as we appreciate your time is valuable, however, at times we may be running behind schedule. Please understand if this occurs it is simply because a patient either in our rooms or in Hospital has required more of our time than we have anticipated. We ask that all patients allow additional time to allow for traffic to ensure they are on time for their appointments and to support us in trying to keep appointments running to time. If you are late for your appointment start time your appointment time may be cut short.
Script Requests
If you require a script outside of an appointment time, this can only be completed if our doctors have seen you for a consultation recently. Alternatively, you can request a script refill from your GP. All script requests outside of an appointment will incur an administration fee and can take up to 7 days to be completed.
